CO-5 denial code: procedure code inconsistent with the place of service
CO-5 says the service and the place it was billed as happening don't go together. Usually the place of service code is wrong. Here is how to match them.
01What CO-5 means
Claim adjustment reason code 5 says: the procedure code or type of bill is inconsistent with the place of service. The service billed isn't one the payer expects in the setting the claim reports.
With group code CO, the patient can't be billed. Correcting the place of service, or the code, and resubmitting usually resolves it.

03Why claims get CO-5
- The place of service defaulted to the practice's usual setting, such as office, for a service performed in a hospital.
- A service only billed in one setting, such as an inpatient or facility-only code, was billed with an office place of service.
- A telehealth visit was billed with an in-person place of service, or the reverse.
- A practice with several locations sent the wrong location's setting.
04How to respond to a CO-5
- Confirm where the service was actually performed, from the note and the schedule.
- Correct the place of service, or the code if the setting was right and the code was wrong.
- Resubmit the corrected claim.
- Fix the default in the charge setup that caused it.
